Sussex Family Practice, S.C. is in its sixth year of offering physical therapy services to their patients and any community members. It is a unique physical therapy department in that there is constant communication that occurs between the physical therapist, physician and patient. It is our hope to offer you a choice of high quality physical therapy in a very timely manner. Physical therapy is a health care profession whose primary purpose is the promotion of optimal health and function through the application of scientific principles to prevent, identify, assess, correct or alleviate acute or prolonged movement dysfunction. Therefore, we are dedicated to helping our patients achieve mobility, strength, stability and balance to reach their highest level of physical ability. With the assistance of a physical therapist and your active participation during sessions and with home programs, you can attain the most of your abilities through exercise and education. As physical therapists, we are skilled at looking at posture, movement dysfunctions and body positioning. Our rehabilitation services include, but are not limited to: musculoskeletal evaluations, postural education, vestibular and balance disorders, occupational health/workman’s compensation injuries, pre/post surgical rehabilitation, sports injury, motor vehicle accidents and movement impairments. We address these issues through therapeutic exercise, postural education, home programs, soft tissue mobilization, joint mobilization, myofascial release, pain relieving modalities, gait training and cervical traction.


During your initial appointment your physical therapist will perform a thorough evaluation to address areas of pain, weakness or concern. You and your physical therapist will work together to set functional goals and outline a plan of care. Our goal for you is to learn how to self-manage your symptoms and become independent. We strive to accommodate your scheduling needs for further appointments.


Most insurance companies mandate that a referral from a physician is needed, but in the state of Wisconsin it is legal to have physical therapy without a referral for the majority of diagnosis. You have a choice to have physical therapy where you choose. As a service to you, we will check your insurance benefits prior to your first visit, so that you will understand what you, as a patient, are responsible for. We have an in staff billing office that will be happy to deal with any questions/concerns regarding insurance.



Meet Your Physical Therapists:


Heike Holzapfel, MPT, CSCS, is a graduate of Carroll College’s Masters of Physical Therapy program with special interests in orthopedic and sports injuries. She is also a Certified Strength and Conditioning Specialist through the National Strength and Conditioning Association. She continues her close ties with the Carroll College Physical Therapy program and assists as needed during labs for the students. She is an active member of both the American and Wisconsin Physical Therapy Associations.




Gina Garlock, PTA, is a graduate of the Milwaukee Area Technical College’s Physical Therapist Assistant program. She has over eight years of experience in geriatrics, home care and outpatient orthopedics combined. Gina has also completed additional training in Myofascial Release and CranioSacral Therapy. In her free time, she assists in youth fitness by volunteering with the Palmrya Park and Recreation department. She also enjoys the patient rapport that physical therapy allows.


Lisa Gehl, MPT, is a graduate of St. Louis University’s Masters of Physical Therapy program. She offers close to 7 years of experience in outpatient physical therapy. Lisa was previously certified through the American Council on Exercise, which allowed her to teach a variety of exercise classes to the public. She continues to enjoy physical activity, especially if it involves the outdoors. Lisa is an active member of the both the American and Wisconsin Physical Therapy Associations.
