Terms of Payment

The initial history and physical exam will be billed to insurance, except patients with PrimeCare or Medicare insurance. Payment for office visits, supplements, medications and monthly laboratory testing is due at time of service.

Policy Regarding Medical Insurance Coverage

In response to requests regarding third party payment for medical expenses incurred at this facility, the following should be noted:

All Sussex Family Practice weight reduction programs are medical treatments of Obesity. Dr. Terrence Hughes and Dr. James Davis are members of the American Society of Bariatrics Physcians and follow this organizations guidelines in administering all weight reduction programs.

Traditionally, most insurance companies do not regard obesity as a diagnosis for which they provide reimbursement for evaluation or treatment.

However, in many cases, insurance coverage is provided if there are concomitant medical conditions which are appropriately treated by weight reduction. Some of these conditions, for example, are diabetes, high blood pressure, elevated blood cholesterol of triglycerides, respiratory distress, arthritis of the weight bearing joints, etc.

As you know, we monitor and manage concurrent and intercurrent medical conditions during the course of your treatment.

We are unable to determine the policy of any particular insurance company regarding reimbursement for your medical expenses. Upon your request, we will submit and itemized statement including the diagnosis as transcribed from your chart. In the final analysis, the decision regarding reimbursement must be made by your insurance carrier.

As in all medical matters, the ultimate responsibility for payment of incurred expenses rests with the patient.
